Hospice Savannah employs a team-oriented approach to expert medical care, pain management, and emotional and spiritual support expressly tailored to the patient's needs and wishes. A non-profit for more than 30 years, the care one receives from Hospice Savannah is rooted in the belief that each of us has the right to die pain-free and with dignity, and that our families will receive the necessary support to allow us to do so.

Hospice Savannah serves anyone regardless of diagnosis, age, race or ability to pay.

 

In-Home Care & Hospice House

In-Home Care & Hospice House

Patients that are unable to be safely or fully cared for at home, may be cared for at Hospice House, a 28-bed, homelike inpatient facility located in Savannah.

We provide three levels of care:

· General Patient Care

· Crisis Care

· Respite Care

Palliative Care

Palliative Care

The Steward Center for Palliative Care, in collaboration with area hospitals, offers comprehensive interdisciplinary care for patients and families dealing with life-limiting illnesses. Palliative care seeks to alleviate suffering and promote a good quality of life for the patient.

· Palliative care specialists treat complex pain and symptoms, handle intensive patient-family communication, and manage after-hours needs and questions.

· Palliative care specialists support the primary physicians' plan of care by making the patient as comfortable as possible at all stages of illness.

Volunteers

Volunteers

Volunteers are the heart of Hospice Savannah. We use them in virtually every area of our organization and count them as a valuable part of our team.

Volunteers average 2-4 hours per week. Every volunteer completes a training course plus additional in-services.

We have a role designed just for you. You may choose one or several positions of interest to you. Full Circle - A Center for Education & Grief Support

Full Circle is a safe place for individuals of any age to find support during their grief journey. It is a place to rest, be reassured, and rejuvenated. Full Circle is also a place for those working with bereaved individuals to find support, education and develop professional liaisons.

Special programs are offered at the Center, and services are also provided to schools, social service agencies and communities of faith throughout the counties we serve. Individual needs vary so support is offered in many traditional and non-traditional ways.

Taking the The First Step

Taking the The First Step

Anyone can make a referral for hospice care. In fact, some patients refer themselves. We will contact the patient's physician to ensure that hospice care is the best choice. Once it has been decided to consider our care, the patient and family are asked to meet with a Hospice Savannah representative to determine what the patient's and family's needs are and explain what services we can provide.

The initial visit can be completed at the patient's home, in the hospital, assisted living facility, long-term care facility, or wherever the patient and family deem convenient. On admission, the patient and family are asked to sign various forms to enter into the program. These forms are required by government or accrediting organizations, and will be explained to you. The Hospice Savannah Team develops a plan of care that meets each patient's individual needs.

 

Our Services

Our Services

Medicare, Medicaid and most private insurance covers Hospice Savannah care, but care is provided regardless of ability to pay. Hospice Savannah covers the cost of all or most aspects of treatment, including:

· Medications and therapies related to the terminal illness

· Music Therapy, Storykeeping, Physical Therapy, Speech Therapy, and Nutritional Counseling as needed

· Durable medical equipment

· Supplies and disposable paper products for patient comfort and caregiver convenience

· 24-hour on-call availability of Hospice Savannah registered nurses and social workers

· Five-day respite stays in Hospice House available to offer caregivers a break

· Follow-up bereavement care and counseling to loved ones during their period of grief

Medical insurance reimbursement only covers a portion of the services that we offer our community. To insure that our unique programs offered to patients and families of Hospice Savannah continue in the future, Hospice Savannah Foundation, Inc. is charged with raising money to fund these programs.
